If you are scheduled for a blood test please do not eat for 8 hours. You can drink water but no other beverages. Please take all your regular medications except those for diabetes while you are fasting. Our standard blood test consists of Complete Metabolic Panel, Complete Blood Count, Lipid Panel, Thyroid Panel and Hemoglobin A1c – these are basic tests designed to asses your general health including your liver, kidney, thyroid function, cholesterol level as well as diabetes screening. We accept most private insurances including Blue Cross Blue Shield, UnitedHealthCare, Aetna, Cigna and Humana. For HMO, we accept NorthShore and Advocate Healthcare. We accept Medicare including most Medicare Advantage products except Humana Gold. We do not take Medicaid, however we do take Meridian which is available to Medicaid qualifying patients in state of Illinois.

Please text front and back of your insurance card to (312) 450-0714 along with your full name and date of birth. Or you can call us at (847) 375-8181. After your insurance has been verified, we will call or text you to make an appointment. We will also need your email to activate your Patient Portal. You can also make an appointment online using the Patient Portal (using the Patient Portal will save you time and you will also be able to see our available time slots to choose from). To save time, please consider printing this NEW PATIENT INTAKE FORM, filling it out and bringing to our office. Please make sure to bring CASH/CHECK for any co-pays you may have (we do not take credit card).
We will need to know all the medications that you are currently taking including doses. If it is easier you can take individual photos of the medication bottles and text us the images to (312) 450-0714 before your visit.
SELF-PAYING PATIENTS:
For patients without insurance we offer the following prices. All self-paying patients should have annual fasting blood test in our office.
